1st Choice Mobile DOT Physicals Service AgreementLast Updated: August 4, 2026

Please read this Service Agreement carefully before reserving or checking into our mobile clinics. Your scheduling locks a dedicated healthcare examiner and diagnostic workspace.

1Acceptance of Terms

By visiting our website, reserving mobile appointments, or utilizing our occupational healthcare services, you explicitly agree to comply with and be bound by these legal Terms and Conditions of service.

2Provided Services

We provide Department of Transportation physical evaluations designed to verify driver health compliance under Federal Motor Carrier Safety Administration (FMCSA) mandates. No secondary medical diagnoses are established, and our examiners do not provide continuous primary care.

3Scheduling, Cancellation & Rescheduling

Online booking reserves a specific slot inside our rotating mobile van. Payment is collected at the time of booking to secure your appointment. We kindly ask that you provide at least 2 hours notice for cancellations or modifications so we can release the slot to other waiting drivers.

4Payment & Collection Policies

All service fees (including the basic $95 exam fee, travel fees, or after-hour fees) are collected directly on-site at your appointment. We accept cash, business checks, major credit/debit cards, and mobile payments. Outstanding balances must be cleared before certificate issuance.

5Medical Compliance Disclaimer

Passing the physical is strictly dependent on meeting standard FMCSA health criteria (vision, auditory, resting blood pressure, vitals, and urinalysis). Our examiner cannot guarantee passing status if you fail to meet standard criteria. We are legally bound to register all results truthfully with the National Database.

6Limitation of Liability

To the maximum extent permitted by law, 1st Choice Mobile DOT Physicals shall not be held liable for any indirect, incidental, or operational damages (such as CDL DMV processing delays or lost transit work) arising from your evaluation outcomes.

Governing Law

These terms are governed and interpreted under the state laws of Florida, without regard to conflict of law guidelines. Any compliance disputes lie within Orange County jurisdiction.

Service Agreement

By booking with 1st Choice, you authorize our occupational examiner to transmit physical evaluation statistics to the FMCSA National Database registry.
